Entry Form – Singles $35.00   Doubles $28.00 per person

• Mixed Doubles will be held Tuesday September 22 - Friday September 25
• Men’s and Women’s Doubles will be held Saturday September 26- Tuesday September 29
• Men's and Women's Singles will be held Wednesday September 30 - Saturday October 3
• 18+ Event
• *Waitlist may sign up, and will be entered if there is space available*
• You MUST be free during the time period of your event.
• Players are responsible for checking match schedule times.
• Draws will be posted 3 days before your event.
• Matches will be USTA scoring (no add, 10-point tiebreaker for the third). 
• Consolation rounds for first match losers in all events, other than round robins.
• Match play will be weekday evenings after 5:30 PM and all day on weekends 10:00 AM-5:30 PM
• Please open your schedule as much as possible. We can work around a few reasonable conflicts during the period of your event. If you're unsure, please contact the tournament directors, Lisa Moldrem & Jeff Eicher to verify that it will work.
• Please list your USTA Number.
